Excelicare and Value Health Partners to Work Together to Facilitate the Journey to Value-Based Care
Partnership will expedite the delivery of customized, comprehensive solutions that meet providers “where they are” and get them to where they want to be.
The Excelicare family of solutions includes robust capabilities for data aggregation, population segmentation, risk stratification, care management, patient engagement, analytics and reporting. As such, Excelicare provides the technology infrastructure and capabilities required to successfully navigate the transition from fee-for-service to value-based care.
To be successful, however, providers must also know how to align their organizations and invest in a culture of continuous improvement. To that end Excelicare has partnered with Value Health Partners to offer comprehensive strategic design, evaluation and implementation services. This includes assistance with the development of:
- a coherent strategy that aligns population needs with payer priorities,
- a long-term roadmap,
- a governance structure, and most importantly,
- a culture of continuous quality improvement.
In the process, Excelicare and VHP are able to meet each provider wherever they are on their journey and get them to where they want to be. In some cases, for example, this means starting by helping the provider leverage Medicare’s existing VBC initiatives — such as chronic care management (CCM) — to provide the incremental funding required to get to MACRA readiness and bonuses, followed by ACO design and performance payment optimization.
Together, Excelicare and VHP not only connect the dots — starting with a de-cluttered, actionable, longitudinal health record and layering on a plethora of capabilities that facilitate action based on that data — but provide the full team necessary to facilitate the journey to value-based care delivery.
