ASPA Connected Community to Expedite Transition to Value-Based Care with Excelicare

The resulting solution will include a full spectrum of population health management capabilities.

ASPA Connected Community, a wholly-owned entity of the Arizona State Physicians Association (ASPA), has selected Excelicare to provide the enabling technology to help member providers

  • deliver improved care and improved outcomes for patients,  
  • effectively involve patients in their own care,
  • develop value-based payer contracts, and
  • preserve the role of the independent provider in care delivery.

Principal among the capabilities that ASPA Connected Community expects to leverage is Excelicare’s exceptional ability to aggregate and normalize data from EHRs, clinical labs, imaging facilities, health information exchanges, claims data and other sources. Additionally, ASPA Connected Community anticipates using Excelicare’s workflow tools, dashboards and analytics tools to facilitate clinical workflows, referrals, clinical and network performance, care management and care coordination.

ASPA Connected Community plans to offer the Excelicare platform to its accountable care organization and clinically integrated network as well as to its affiliated entities – ASPA itself (which operates as an independent physician association) and ASPA Empowered Healthcare Services, LLC (a sister organization that provides clinical support services and selected clinical services).
